Chev Chelios (Jason Statham), the hired killer with a day to live from Crank, is back. He survived falling out of a helicopter only to be used as a vessel for farming organs. His massive heart is the first organ to go, replaced by a battery-operated artificial heart to keep him temporarily alive. The battery-life that keeps his new heart pumping only has an hour of juice before it dies (and him along with it), forcing Chelios to give himself a charge of electricity every few minutes so he has enough time to find the ones responsible and get his heart back, leaving a path of death and destruction in his wake.
